Newly Diagnosed - Your First Visit

At your first visit, we will create a permanent health record. We will ask you to bring the completed forms listed below or complete them when you arrive and will expect to have received copies of health records from all other physicians you have seen. In addition, please bring copies of your Living Will or Power of Attorney for Healthcare, if you have them. We will take copies of your insurance cards and collect co-pays, as well.

Prior to your first visit to MIHOA, please fill out the forms below and bring them with you to your first appointment.

Your physician will go over your diagnosis and order any other tests that may be necessary to finalize your diagnosis and set-up a treatment plan.

Depending on the reason for your visit, your physician may order lab work for you which can be done at our office before you leave.

Occasionally, in unusual or complicated situations, we can deliver better care by seeking the opinions of another physician. Thus, your physician may suggest a consultation with another specialist and may refer you to his/her care.
